Waiting List Application

The Waiting List Application for 2010 is now available. Before signing up, please read the following before proceeding:

  • You may submit a Waiting List Application if you did not participate in the October 2009 Lottery Drawing System
  • You may submit a Waiting List Application if you participated in the Lottery Drawing System but did not receive a time slot. (Please check with the Programming Department to see if you are already listed)
  • You may submit a Waiting List Application if you are interested in having an ongoing series program (weekly or monthly)
  • DO NOT FILL THIS OUT IF YOU ARE SUBMITTING SPECIAL PROGRAMS 

Like all QPTV submissions, we require proof of residency. The following combinations of proof are acceptable:

  • New York State Driver's license and two current utility bills (within the last six months.)
  • New York State Photo I.D. (a.k.a. "non driver's license") and two current utility bills (within the last six months.)
  • A United States or foreign issued Passport and two current utility bills (within the last six months.)
  • A City, State, Federal or Employee Photo ID and two current utility bills (within the last six months.)
  • Current College or High School ID with two proofs of address (must be 18 years of age or older). -
  • Copy of 501(c) 3 (Not-For-Profit organizations only.)

You must provide proof of residency for your application to be processed.
